Community Health Worker Support

A community health worker is available to help with navigation and follow-up, not direct medical care.

  • Understanding next steps after your visit

  • Follow-up testing or lab referrals

  • Insurance questions or cost concerns

  • Enrolling in assistance programs

  • Transportation support (such as Lyft rides)

  • Finding a provider for ongoing care
