Community Health Worker Support

A community health worker is available to help patients and providers navigate care before and after the pharmacy visit.

Contact the community health worker for help with:

  • Understanding next steps after a pharmacy visit

  • Follow up lab testing or referrals

  • Insurance questions and cost concerns

  • Enrollment in assistance programs such as PrEP AP

  • Transportation support, including Lyft rides

  • Connecting to primary care or specialty providers
